Appointments Because appointment hours are reserved for you, you may be assessed a fee if 24-hour advance notice is not provided to cancel or change your appointment. The missed appointment fee is not covered by insurance.
Medication refills If you are in need of a refill on your medication, please contact your pharmacy and have them send a Refill Authorization Request to us. Our prescribers handle medication refills during regular business hours, Monday through Thursday. Please allow us 24 hours to process your refill request.
Our practice utilizes the Virginia Prescription Monitoring Program to obtain patient medication history. The prescription monitoring program collects prescription data for Schedule II-IV drugs into a central database which can then be used by limited authorized users to assist in deterring the illegitimate use of prescription drugs. Please contact us for more information on this program.
Confidentiality Information about you can only be released by this office with your written consent. Your consent can be revoked at any time. Substance abuse records are covered by Federal Law (CFR-42, Part 2), and cannot be released without your written consent, except by court order. There are some situations when information you provide to your physician or therapist is required by Virginia law to be reported to the appropriate agency. For example, if a child, elderly or disabled person is being physically abused, or if a patient is threatening bodily harm to self or others. Mental health professionals are also required to report ethical/legal violations by prior therapists.
